MICRONEEDLING (a.k.a. collagen induction therapy)
Microneedling is the fastest growing aesthetic procedure and, depending on depth of needle penetration, is performed
by estheticians (up to 0.3 mm) and medical personnel (0.5 mm and longer needles.) Professionals use needle rollers or
reciprocating pen-like devices with disposable cartridges containing multiple needles. Treatment with all devices
creates thousands of microscopic perforations in the superficial layers of the skin. Note: some state regulations do not
allow estheticians to perform microneedling procedures.
Microneedling is effective in skin rejuvenation due to enhanced penetration of topical actives (all needle lengths) and
because longer needles create injury within the keratinocyte layer, or deeper dermal layers. The procedure induces a
natural healing response without the trauma caused by ablative treatments that destroy tissue.
Cellular injury from longer needles initiates cascades of endogenous cytokines that promote cellular proliferation and
collagenesis. Injury into deeper layers also creates capillary bleeding that results in platelet growth factor and cytokine
release that enhance certain aspects of the healing process but also promote an inflammatory response. The use of
topical autologous platelet rich plasma is popular although this effort to provide growth factors is a two-edge sword.
Platelets are heavily loaded with pro-inflammatory bio-signals, exacerbating the normal inflammatory response. Skin
health experts agree inflammation in the skin, particularly on a chronic basis, is best avoided, making the prudence of
deliberately creating a heightened state of inflammation a questionable practice.
Microneedling is effective in producing smoother, firmer, softer skin with reduced pigment abnormalities, and
diminished fine and coarse lines. It is commonly performed as a series of treatments. When for enhanced penetration
of actives (aesthetic), treatments can be frequent (every few days to a week or two apart.) When longer needles are
used, the healing process is more extensive and takes four weeks to complete. A series of four to six medical dermal
needle sessions at monthly intervals can produce aesthetic changes that rival that of fractional CO2 laser resurfacing.
